- Click on ‘Apply for the Complex PTSD Provider Certification’ below
- Complete the ‘Certification Application’ form , use the ‘Next’ button to go through all the pages (Personal Details, Education, Training, Clinical Experience and the Submission page).
- Items marked ‘Required’ must be completed in order to move to the next page
- Upload the Documentation required in each Section
- Complete the Attestation required in each Section
- Submit the form on the final page
- Complete the remaining questions below
